When you become eligible for a Centrelink benefit you receive a Concession Card within 2 weeks:

You may get a Pensioner Concession Card or Health Care Card if you get specific payments from Centrelink. This card gives you access to cheaper prescription medicines under the Pharmaceutical Benefits Scheme. State and territory governments and local councils may also offer concessions for energy and electricity, healthcare, public transport, and rates. The concessions can vary so it is best to check with your state and territory government.
If you get an eligible payment from Centrelink, you don’t need to apply for the card - Centrelink posts one to you. You can also access the card through the Express Plus Centrelink mobile app by selecting ‘Digital Wallet’.
